Information About Your Family

If there are children involved the court will require this information. This means any children born to or adopted by you and the other party.

It is required that we provide information about who the child has lived with and where they have lived for the last five years.

Monthly Expenses

Whenever child support can possibly be an issue, or when the court must decide issues of financial matters, they require a great deal of information on your finances. If you don't have a particular expense, mark the field with a "no". This way we don't have to ask you if you missed a question and slow down the preparation of your documents.

We can only make you look as good as we can with the information you provide to us. If you don't, it wont be input into the paperwork and the judge wont have the opportunity to consider the information.

This is your opportunity to provide specific information regarding your case. Don't forget, the judge bases their decision on the information in the documents you file with the court. Please include any dates and specific timelines about the situation and explain why they’re important. If there have been any significant changes in your circumstances (lost job, child abuse, other party now in jail) please tell us about them so we can help to provide that information to the courts.
